Topic: Baby Boomers Will Challenge and Change the Health Care Delivery System
Presentation will outline the patient, medical professional/caregiver and chronic
disease statistical data related to the baby boomer generation and initiatives being taken by the
medical community and government to deal with their impact. Discuss how relational coordination
and communication among institutional, community and in-home based services will facilitate the
continuum of care. Conclude what measures will need to be taken to meet the complex demand and
result in better outcomes for patients and greater job satisfaction for medical professionals.
